I play the piano, but I’m not very good at it.
上手（じょうず） is a na-adjective.
To make it negative, we say:
→ 上手じゃありません (not good at)
Also, this sentence has あまり.
あまり is usually used in negative sentences.
So we need a negative form.
That’s why the correct answer is:
→ (上手)じゃありません。
Other Choices:
〜でした
= past tense of na-adjectives
〜くないです
