〜てほしい typically follows the structure [person]に[verb in te-form]ほしい.
Since 医者になる is a fixed phrase, we convert it into te-form as a whole, resulting in [あに]に[医者になって]ほしい. The particle に appearing twice is not an issue, as each serves a different function.
